The 2017 Wellness Rewards Program window ended on November 1, 2016. If you and your spouse completed the requirements and earned your Wellness Reward for 2017, your reward will be deposited in your healthcare expense account in January 2017! Don’t be concerned if your Wellness Reward amount is not displayed in the Total Rewards Café (enrollment system) in 2016. Your 2017 Wellness Reward amount will not appear in the system until January 2017.

  1. Am I eligible to receive the 2018 Wellness Reward?

    You must be an active associate and enrolled in a Sysco-sponsored medical plan in 2017 and 2018 to be eligible to receive your 2018 Wellness Reward.

  2. Is my spouse eligible to receive the 2018 Wellness Reward?

    If you are eligible and you cover your spouse under your Sysco-sponsored medical plan in 2017 and 2018, then your spouse is eligible to participate and earn a reward in the amount of $200.

  3. What if my spouse is also a Sysco associate, are we both eligible for the associate Wellness Reward in the amount of $400?

    If your spouse is also a Sysco associate and he or she is covered under your Sysco-sponsored medical plan in 2017 and 2018 (associate & spouse coverage), then your spouse is only eligible for the spouse Wellness Reward in the amount of $200. Each individual may only earn a single Wellness Reward.

  4. What if my spouse is also a Sysco associate but he or she is covered under his or her own Sysco medical plan?

    If your spouse is also a Sysco associate and he or she enrolls in a separate Sysco-sponsored medical plan in 2017 and 2018 (not associate & spouse coverage under your Sysco medical plan), then your spouse is eligible for the associate Wellness Reward in the amount of $400. Each individual may only earn a single Wellness Reward.

  5. Do I have to be enrolled in a Sysco-sponsored medical plan in 2017 to participate in the wellness program?

    Yes. You and your spouse must be enrolled in a Sysco-sponsored medical plan in 2017 and 2018 to be eligible to receive your 2018 Wellness Reward.

  6. What if I am currently enrolled in a Sysco-sponsored medical plan for 2017 but I choose to waive my coverage in 2018?

    You must be enrolled in a Sysco-sponsored medical plan in 2017 and 2018 to receive your 2018 Wellness Reward. If you actively waive your Sysco medical plan coverage for 2018 during the 2018 benefits enrollment period, you will not receive the Wellness Reward in January, even if you have completed the requirements to earn your 2018 reward.

  7. Why do I have to be enrolled in a Sysco-sponsored medical plan to earn the Wellness Reward?

    The 2018 Wellness Reward that you earn is deposited into your healthcare expense account, and you must be enrolled in a Sysco-sponsored medical plan in 2018 with a healthcare expense account to receive the reward.

  8. What is a healthcare expense account?

    Depending on which Sysco-sponsored medical plan you are enrolled in, you have one of two types of health expense account—a Health Savings Account or a Health Reimbursement Account. You can use the money in your account to pay for out-of-pocket medical expenses. Both of these types of accounts allow you to roll your unused money over from year to year without losing it.

  9. What do I need to do to earn the 2018 Wellness Reward?

    You and/or your spouse have two options to earn your reward: you may get an annual physical with your primary care doctor or a biometric screening at Quest Diagnostics. If you choose to do the biometric screening through Quest Diagnostics (or have already received one in 2017), you will automatically earn the 2018 Wellness Reward. If you earn your reward by getting an annual physical at your doctor’s office, you need to complete the “I Got My Physical” Form by November 1, 2017, to receive the 2018 Wellness Reward. Remember, you and your spouse must be enroll in a Sysco-sponsored medical plan in 2017 and 2018 to be eligible to receive your 2018 Wellness Reward.

  10. Why do I need to get an annual physical?

    Because your health is so important—to you and to us! Getting an annual physical and establishing a relationship with a doctor is the single most important step you can take to start improving your health today. It will give you a clear picture of your current health status and a roadmap to help you achieve your health goals. We feel it is so important that we are adding it as an option to fulfill the Wellness Reward requirements that reward you and your covered spouse for your efforts to maintain your health!

  11. How much is the 2018 Wellness Reward?

    If you get an annual physical or a biometric screening and submit your “I Got My Physical” Form (if applicable), you will earn $400. If your covered spouse completes either of the required actions too; you will earn an extra $200. That’s a total of $600, deposited into your healthcare expense account in January 2018 to use for qualified out-of-pocket medical expenses, including copays.

  12. Can I still earn my full 2018 Wellness Rewards if my screening numbers are not in the normal ranges?

    Yes. Your 2018 Wellness Reward is determined solely by your completion of an annual physical with a primary care doctor or a biometric screening at Quest Diagnostics. You are not required to achieve healthy ranges or participate in health management programs to receive your reward. However, we do encourage you to review your screening results with your primary care doctor and discuss ways to help you improve or maintain your health.

  13. Why is it important for me to see a primary care provider?

    Changes to the healthcare environment such as the Affordable Care Act and a growing elderly population, may result in fewer primary care doctors practicing, and even fewer that have capacity to take on new patients. So, it is more important than ever to establish a relationship with a primary care doctor that you trust so you will have someone to call if and when you do need care.

  14. How do I earn my 2018 Wellness Reward?

    You can earn your 2018 Wellness Reward one of two ways:

    • Get an annual physical at your primary care doctor’s office. Then, let us know that you got your physical by completing the “I Got My Physical” Form, ─OR─
    • Get a biometric screening through Quest Diagnostics, where your participation is automatically reported.
  15. Does my spouse have to get an annual physical or biometric screening too?

    If your spouse is covered under your Sysco-sponsored medical plan in 2017 and 2018, he or she may also get an annual physical with a primary care doctor or a biometric screening at Quest Diagnostics and submit the “I Got My Physical” Form (if applicable) by November 1, 2017, to earn a Wellness Reward in 2018.

  16. Will I earn a Wellness Reward if I choose not to get an annual physical or biometric screening but my covered spouse receives one?

    If your spouse is covered under your Sysco-sponsored medical plan in 2017 and 2018, and he or she completes the reward requirements by November 1, 2017, you will receive $200, deposited into your healthcare expense account in 2018. However, you will not receive the $400 associate Wellness Reward, unless you also meet the requirements. Don’t miss out on the additional $400!

  17. I'm covering a child on the Sysco medical plan. Does he or she need to get an annual physical or biometric screening for me to earn my reward?

    No. Although everyone should get a physical as part of staying healthy, children are not required to meet the requirements in order for you to earn my 2018 Wellness Reward and children are not eligible to earn a separate award.

  18. Does my annual well-woman exam qualify me to earn the reward?

    Yes. You can see your primary care doctor for an annual physical or, if a woman, your annual well-woman exam counts too! Simply let us know that you had your visit by completing the “I Got My Physical” Form.

  19. Does my DOT physical qualify me to earn the reward?

    No. Your DOT physical is for job-related purposes only and does not qualify you to receive a 2018 Wellness Reward. You must get an annual physical with your doctor or a biometric screening through Quest Diagnostics to receive your 2018 Wellness Reward.

  20. Do exams given in the Emergency Room, urgent care facilities or in-store clinics (CVS Minute Clinic® or Walgreens Healthcare Clinic) qualify me to earn the reward?

    No. If you have not already received a screening or physical in 2017, you must get an annual physical with your doctor or a biometric screening through Quest Diagnostics to receive your 2018 Wellness Reward.

  21. How much does an annual physical or biometric screening cost?

    If you are covered under a Sysco-sponsored medical plan in 2017, Sysco pays 100% of the cost of in-network preventive care for you and your covered spouse. So whether you receive a biometric screening through Quest Diagnostics or an annual physical at your in-network primary care doctor or OBGYN, these services are covered at 100% through your Sysco-sponsored medical plan. Print the “Letter to your Doctor” and give it to your doctor when you receive your annual physical to ensure that your doctor codes your visit correctly (as preventive).

  22. How will I know that my doctor is coding my annual physical as preventive?

    To ensure that your doctor codes your annual physical as preventive, print the “Letter to your Doctor” and present it to your doctor on the day of your appointment. Also, when scheduling your annual physical, make sure you tell the doctor’s office that you are coming in for your preventive annual physical (age/gender appropriate preventive care screening).

  23. How will I receive my 2018 Wellness Reward?

    Sysco will automatically deposit your reward into your healthcare expense account, even if you haven’t opened one as part of the benefits enrollment process this fall. Based on the Sysco-sponsored medical plan you select, you will be eligible for either a Health Savings Account or Health Reimbursement Account that will be funded in January with up to $600 to spend on qualified out-of-pocket medical expenses, including copays.

  24. What is the deadline to earn the 2018 Wellness Reward? When do I need to get my annual physical or biometric screening?

    You and your spouse need to get an annual physical with a primary care doctor or a biometric screening at Quest Diagnostics and submit your “I Got My Physical” Form (if applicable) by November 1, 2017, to earn your 2018 Wellness Reward. Don’t forget, you and your spouse must be enrolled in a Sysco-sponsored medical plan in 2017 and 2018 to earn your 2018 Wellness Reward.

  25. What if I have already completed my biometric screening or annual physical for 2017?

    If you completed a biometric screening through Quest Diagnostics or received an annual physical with your doctor in 2017, you have already earned your 2018 Wellness Reward. If you receive an annual physical at your doctor’s office, make sure you submit the “I Got My Physical” Form so we register that you received your physical. If you received a biometric screening at a Quest Diagnostics in 2017, your participation was automatically registered.

    Now, go the extra step, even though it’s not required: take your biometric screening results to your primary care doctor and discuss steps that you can take to improve or maintain your health. Remember, you and your spouse must be enrolled in a Sysco-sponsored medical plan in 2017 and 2018 to be eligible to earn your 2018 Wellness Reward.

  26. What if my doctor orders lab work at a blood lab other than Quest Diagnostics in 2017?

    If your doctor orders lab work at a blood lab other than Quest Diagnostics in 2017, you are still eligible to earn your 2018 Wellness Reward. Simply submit** the “I Got My Screening at a Non-Quest Location” Form before November 1, 2017, to report your screening.

  27. If I have already completed a biometric screening and earned my 2018 Wellness Reward, why do I need to see a primary care doctor?

    If you’ve completed your biometric screening, seeing a doctor is not a requirement to receive your 2018 Wellness Reward, but we encourage you to take this additional step. Why? Because we care about you—and you should, too. We know many of you have physical jobs and we want you to have a partner in managing your health. Even if you are young, there are benefits to establishing a relationship with a primary doctor and creating a baseline of knowledge about your current health status.

  29. If I prefer not to get an annual physical, how do I schedule an appointment to complete my biometric screening through Quest Diagnostics?

    Follow these easy instructions:

    • Go to my.questforhealth.com
    • Even if you have used Quest before, login as a New Participant, and use the Registration Key Sysco.
      • Your unique ID is the last 4 digits of the associate SSN, plus MMDDYYYY of the associate’s birth.

        Example:
        Associate SSN: 123456789
        Date of Birth: November 12, 1967
        Unique ID for the associate: 678911121967
        Unique ID for the spouse: 678911121967S (note, the spouse uses the associate’s information, plus an “S” at the end).

  30. Will Sysco see information about my biometric screening?

    No. Your health information is private and protected under federal law. The Sysco Plan will only know that your biometric screening took place. Any information discussed will remain between you and your doctor.

  31. How can I get a copy of my most recent biometric screening results to share with my doctor?

    1. Log on to my.questforhealth.com.
    2. Even if you have used Quest before, login as a New Participant, and use the Registration Key Sysco.
      • Your unique ID is the last 4 digits of the associate SSN, plus MMDDYYYY of the associate’s birth.

        Example:

  32. How can I be sure that Sysco receives notification from Quest Diagnostics that I have completed my biometric screening? How can I make sure I earn my 2018 Wellness Reward?

    Quest Diagnostics will mail you a “My Guide To Health” report that contains your biometric screening results within 7-10 business days after you complete your screening. If you do not receive a “My Guide To Health” report mailed to your home in this timeframe, you may contact Quest Diagnostics to be certain that they have a record of your biometric screening. If Quest has your screening on file, then the Sysco Plan has been given notice that the screening requirement has been satisfied.

  33. If I self-report my annual physical using an “I Got My Physical” Form, how does the Sysco Plan know that I completed my physical?

    You will report this activity, and your Wellness Reward request will be verified by our medical plan carrier, who processes the medical claim for the doctor’s office visit. If you are not currently covered under a Sysco medical plan you may be asked to provide proof of your annual physical by supplying an Explanation of Benefits or other summary documents.
