Benefits Available After You Leave Sysco
If you leave Sysco, you may be eligible to continue certain benefits, however, you will be responsible for the full cost. Visit the Contacts page to contact the benefits provider directly.
Health Savings Account (HSA)
You may continue to use available HSA funds for qualified medical expenses; however, you may not be able to contribute to your HSA. You will be responsible for any applicable fees. Additional information can be found here.
MetLife Coverage (Life, AD&D, and Pet Insurance)
You can continue the following MetLife coverages: Life and AD&DÂ Insurance, Supplemental Life and Voluntary AD&D Insurance, and Pet Insurance.
Call 1-800-638-6420 or go to metlife.com/continuation for your Life Insurance and AD&D options. Call 1-800-438-6388 for information about Pet Insurance.
Accident, Critical Illness, and Hospital Indemnity Plans
If you wish to continue your Accident, Critical Illness, and/or Hospital Indemnity Plan coverage, complete this form and provide it to Aetna.
401(k)
You will continue to have access to your 401(k) account, though you will no longer be able to make contributions. You can see the vesting schedule, find forms, and manage your account online at NetBenefits.com or call Fidelity at 1-800-635-4015.
Legal Insurance, Identity Theft Protection, and Home & Auto Insurance
You may also continue these benefits. Call the providers for information on continuing these coverages.
COBRA
The Consolidated Omnibus Budget Reconciliation Act (COBRA) allows you and your enrolled dependents to maintain your medical, prescription drug, dental, and vision coverage after you leave Sysco. You will be responsible for the full premium amount and must enroll in coverage via the online Sysco Benefits Center within 60 days of your termination date. You will also continue to have access to the EAP for 60 days, which is offered at no cost to you. If you are eligible for COBRA, a notification will be mailed to you within 30 days of your last day of employment. You can find more information about COBRA here.
When Coverage Ends
The date you or your covered dependents’ benefits coverage ends varies based on the reason you leave Sysco or how your employment ends.

Your dependent turns age 26
The last day of the month your dependent turns age 26 (at 11:59 p.m.).
You have a qualifying life event that involves ending your coverage (i.e., like getting married and joining your spouse’s plan)
The date of the qualified status change. Learn more about qualifying life events here.
You retire from Sysco
- ‍If you have met the 10 years of service requirement for retiring, coverage ends the last day of the month you retire. ‍
- If you haven’t met the retirement eligibility requirements, coverage ends on your last day of employment.

You pass away
On the date of your date of death; coverage for enrolled dependents remains in effect until the end of the month.
You leave Sysco
Your last day of employment at 11:59 p.m.‍; you will be eligible to continue health plan coverage through COBRA for up to 18 months.
FSA Balances
Flexible Spending Accounts (FSAs)Â are "use it or lose it," so if you leave Sysco and you have not incurred any expenses as an active colleague, your FSA funds will be forfeited.
You will have 90 days from your termination date to submit claims for reimbursement of services incurred during your active employment.
