Medical Plan Look-Up

Not sure which medical plan you’re enrolled in for 2025? To find your plan, go to the Total Rewards Café.

ID Cards

If you enrolled in a Sysco-sponsored national medical plan for the first time, changed your medical plan election, or added or changed dependents for 2025, you will receive a medical ID card from Aetna in early January. You will not get a new medical plan ID card if you didn't change plans for 2025. You can download your ID card on the Aetna app. Learn more here.

In-Network Providers

You’ll get the best cost for care if you see an in-network provider, so make sure your current medical, dental, behavioral health, and other providers are in the Aetna network before using your benefits. Find an in-network provider here.

Add/Update Your Beneficiaries

Make sure your beneficiaries are added to your Life and Accidental Death and Disability (AD&D) coverages and 401(k) plan so your benefits go to the right people in the event of an unexpected loss. You can make changes to your AD&D beneficiaries at any time in the Total Rewards Café. Go to Fidelity to change your 401(k) beneficiaries.

401(k) Contribution Limits

The IRS limits the combination of your pre-tax, after-tax, Roth 401(k), and Company contributions to $23,500 in 2025. This includes any before-tax contributions you made to another employer plan within the same year.

Catch-up Contributions

  • If you are age 50 to 59 or over 64, you’re eligible to make additional pre-tax contributions (unmatched) into your 401(k) up to a maximum of $7,500 in 2025.
  • If you are age 60 to 63, you’re eligible to make additional pre-tax contributions (unmatched) into your 401(k) up to a maximum of $11,250 in 2025.
  • You can change your contribution at any time.

Learn more here.

Health Savings Account (HSA) Contribution Limits

The 2025 IRS HSA contribution limits are $4,300 for individual coverage and $8,550 for family coverage levels. If you are age 55 or older, you can contribute an additional $1,000. Sysco contributes $250 for individual and $500 for family coverage to your HSA, which counts toward the IRS contribution limit. You can change your contribution at any time in the Total Rewards Café.
