Eligibility

Full-time, active colleagues in the U.S. are eligible for Sysco's group benefits (medical, dental, vision, life, AD&D, long-term disability, and flexible spending accounts). You may also cover certain family members, including your legal spouse (unless otherwise required by law) or domestic partner, and your dependent child(ren). Learn more here.

Dependent Verification

If you add dependents to your coverage, you will need to provide dependent verification (i.e., birth certificate, proof of marriage and joint ownership paperwork such as a mortgage if the date of marriage is not within the past 12 months, adoption or legal guardianship for a child,or a domestic partner affidavit) within 45 days of enrolling to validate the relationship with your eligible dependents. Our Dependent Verification Center will request documentation from you. You can find additional details about the verification process here. Falsification of dependent eligibility may result in disciplinary action, up to and including loss of benefits or termination of employment.

Health Savings Account Transfer

If you currently have an Edward Don & Company HSA through Bank of America, you can choose to transfer your current HSA balance to Inspira, Sysco’s HSA administrator.

After you are enrolled in the Sysco HSA, the Inspira website will give you instructions on how to transfer your balance.

Please note that you are responsible for any account fees if you elect to keep your HSA with Bank of America.

Transition of Care

Transition of Care services are available if you are currently in an active course of treatment that will be disrupted due to a change in your medical plan. Some examples include a scheduled surgery, pregnancy, or ongoing treatment for a serious illness or certain behavioral health conditions. Transition of Care services allow you to continue to work with your current care team for a designated period of time.

To initiate Transition of Care coverage, you will need to complete a transition of care form which you can find here.

If You Do Not Enroll

If you do not enroll, the only Sysco benefits you will have as of January 1 are the following company-paid benefits:

Your next opportunity to enroll in benefits will be next year's Annual Benefits Enrollment unless you experience a qualifying life event.

Add Your Beneficiaries

Make sure your beneficiaries are added to your Life and Accidental Death and Disability (AD&D) coverages and the 401(k) plan. If you do not elect beneficiaries, it will cause delays in distributing benefits or account access in the event of your death. You can make changes to your beneficiaries at any time in the Total Rewards Café.

Aetna Provider Search Tools

Many of your benefits are administered by Aetna. Aetna has one of the largest networks in the country, with 1.2 million health care professionals, over 690,000 primary care doctors and specialists, and over 5,700 hospitals.

It’s important to make sure your medical, dental, behavioral health, and other current providers are in the Aetna network before you begin using your benefits.

Click here to see if your provider is in the Aetna network. You will be prompted to select a plan name. The plan names will include the medical plans below:
