MetLife is Sysco's Life and AD&D carrier.

As a first step, colleagues or HR should contact the Sysco Benefits Center by calling 800-55-SYSCO. The Sysco Benefits Center will contact MetLife to begin the notification process. Colleagues may contact MetLife at any point with questions however claims will not be processed until all information is provided by the Sysco Benefits Center.

You can find instructions for filing claims here. You can submit documents for a claim three different ways:

  • Email: Scan documents and email them to Lifeclaimssubmit@metlife.com
  • Fax: 1-570-558-8645
  • Mail: MetLife Group Life Claims, P.O. Box 6100, Scranton, PA 18505-6100

Sysco pays the full cost of your Basic Life and Basic AD&D coverage.

Paycheck costs for Voluntary Life and AD&D Insurance are based on the colleague's age. The cost of coverage can be found by logging in to the Total Rewards Café.

Sysco pays the full cost of your Basic Life and Basic AD&D coverage. These benefits are automatically yours as an eligible Sysco colleague.

  • Basic Life Insurance provides coverage equal to 1.5x your previous year‘s eligible earnings (rounded up to the nearest $1,000) up to a maximum of $225,000.
  • Basic Accidental Death & Dismemberment (AD&D) provides coverage equal to your previous year‘s eligible earnings (rounded up to the nearest $1,000) up to a maximum of $150,000 if you lose your life or suffer dismemberment as the result of an accident.

Sysco will automatically provide you coverage equal to your previous year’s eligible earnings up to a maximum of $150,000. If you choose Voluntary AD&D you can elect $10,000 up to $500,000; however, coverage amounts cannot exceed 10x your annual earnings. For your family, you can elect coverage from $10,000 to $500,000. Again, coverage amounts cannot exceed 10x your annual earnings.

The 2018 Wellness Rewards Program window ended on November 1, 2017. If you and your spouse completed the requirements by November 1, 2017, and earned your Wellness Reward for 2018, your reward will be deposited in your healthcare expense account in January 2018! Don’t be concerned if your Wellness Reward amount is not displayed in the enrollment system on the Total Rewards Café in 2017. Your 2018 Wellness Reward amount will not appear in the system until January 2018.

Please note: If you and your spouse completed the requirements early — by October 1, 2017 — and did not choose to receive your reward a different way during 2018 Benefits Enrollment, you will receive your reward as a credit to the medical plan premium taken from your paycheck each pay period in 2018.
